Mission Statement:
M4A’s Assisting with Independence and Mobility (AIM) program is a community service project initiative providing safe and healthy independence and mobility solutions for elderly and disabled individuals living within Blount, Chilton, Shelby, St. Clair, and Walker Counties.
Community Service Project Examples:

- Yard Cleanup or Maintenance
- Other Low-Cost Home Modifications and Maintenance (By Approval)
Although community service projects offered have no cost to clients, the AIM program relies on the generosity of the community’s donations and volunteer efforts. Client and volunteer cost-sharing is always encouraged and appreciated.

Blount County Projects
JRHR9 – Clients are an elderly couple living in rural Locust Fork. Both husband and wife suffer from multiple medical complications, and they are on a fixed-income. For this reason, they have expressed the need for assistance with installation of a wheelchair ramp and grab bars. These items will assist the couple with maintaining health, safety, and independence. Any assistance is greatly appreciated.
JRHR10 – Client is a 66-year-old woman who lives alone in the Trafford area. She has multiple medical ailments that inhibit her ability safely ambulate in-and-out of her shower including arthritis, asthma, COPD, depression, high blood pressure, osteoporosis, and cataracts. Client is on a low monthly fixed income; therefore, she has expressed the need for assistance purchasing a grab bar for her bathroom. Any assistance is greatly appreciated!
JRHR14 – Client is a 71-year-old widow who lives alone in Remlap. She has multiple medical ailments that inhibit her ability safely ambulate in-and-out of her home including arthritis, diabetes, depression, high blood pressure, and visual impairments. For this reason, she has expressed the need for a wheelchair ramp, and potentially a shower chair, as well. Client is on a monthly fixed income; therefore, she is unable to financial afford to pay for the project requests, so any assistance is greatly appreciated!
JRHR64 – Client is a 90-year-old widow who lives with her relatives in the Oneonta community. She has multiple medical ailments that inhibit her ability safely ambulate in-and-out, as well as around, her home. These ailments include Alzheimer’s disease, arthritis, asthma, AFIB, diabetes, high blood pressure, and visual impairments. Her family cares for her the best they can, but household financial limitations inhibit the family’s ability to pay for a wheelchair ramp and grab bars for shower would substantially increase the health and safety for the client and her family. Additionally, the client has requested assistance with yardwork/debris cleanup. Any assistance is greatly appreciated!
JRHR18 – Client is a 67-year-old female who lives alone in the Hayden community. She has multiple medical ailments that inhibit her ability safely ambulate in-and-out, as well as around, her home. These ailments include an amputation, arthritis, depression, heart disease, and history of a stroke. Client is on a low monthly fixed income, so she in unable to financially pay for a wheelchair ramp and grab bars for shower would substantially increase the health, safety, and independence for the client. Additionally, the client has requested assistance with repairing some of her broken windows. Any assistance is greatly appreciated!
JRHR68– Client is a 71-year-old female who lives with her spouse in the Blountsville community. She has multiple medical ailments that inhibit her ability safely ambulate in-and-out of her home. These ailments include arthritis, COPD, depression, diabetes, high blood pressure, knee replacement, heart complications, and history of a stroke. Client’s household is on a monthly fixed income, but they might be able to moderately assist with paying for a wheelchair ramp. The addition of a ramp would substantially increase the health, safety, and independence for the household. Any assistance is greatly appreciated!
JRHR54– Client is a 71-year-old widow living with a relative in the Altoona community. She has multiple medical ailments that inhibit her ability safely ambulate in-and-out of her home. These ailments include arthritis, hearing impairments, heart complications, high blood pressure, visual impairments, scoliosis, and breathing issues. Client’s household is on a monthly fixed income, so they are unable to pay for a wheelchair ramp. Additionally, the client has asked for a faucet to be hooked up that she is unable to pay to be installed. Both projects would help to increase the health, safety, and independence for the household. Any assistance is greatly appreciated!
Chilton County Projects
JRHR13 – Client is a 71-year-old man who lives in Maplesville in a small camper next door to family members. He has multiple medical ailments including vision and hearing impairments, heart problems, and dementia. Family members and healthcare professionals made a referral for the client’s need for a wheelchair ramp, but due to his financial situation, he is unable to pay for a ramp to be build. For this reason, the family is seeking assistance from volunteers to assist in building the client a wheelchair ramp. Any assistance is greatly appreciated!
JRHR17 – Client is a 79-year-old woman who lives in the Maplesville area with her niece (caregiver). She has multiple medical ailments including vision impairments, history of cancer, and dementia. These ailments inhibit her ability to function in her environments. Due to financial constraints, her caregiver has asked for assistance with building a wheelchair ramp, as well as handrail repairs on the exterior of her home. Any assistance is greatly appreciated!
JRHR29 – Client is a 67-year-old widow who lives in the Clanton area. Client’s home is an older home in need of many repairs, including window repairs and front and rear door replacement. Due to financial constraints, she is unable to pay for any repairs at all, so she has asked for assistance with completing these projects. Any assistance is greatly appreciated!
JRHR36 – Client is a 68-year-old female who lives in the Clanton area. Client experiences multiple medical ailments including COPD, asthma, history of cancer, and visual impairments. She with her niece (caregiver). Client’s home is an older home in need of many repairs, including interior and exterior painting. Due to financial constraints, she is unable to pay for any projects to be completed at this time, so she has asked for assistance with completing these projects. Any assistance is greatly appreciated!
JRHR37 – Client is a 92-year-old widow who lives in the Maplesville area. Client’s home is an older home in need of many repairs, including exterior door replacements and a ramp repair/replacement. Due to financial constraints, she is unable to pay for any repairs on her own, so she has asked for assistance with completing these projects. Any assistance is greatly appreciated!
JRHR41 – Client is a 51-year-old female who lives in the Clanton area. Client’s home is an older home in need of many repairs, including painting, new paneling, and what she describes as “TLC”. Client has multiple medical ailments including arthritis, asthma, cancer, heart complications, visual impairments, and an amputation. Due to financial constraints, she is unable to pay for any of the projects on her own, so she has asked for assistance with completing these projects. Any assistance is greatly appreciated!
JRHR69– Client is an 83-year-old female widow living with relatives in the Clanton area. Client has multiple medical ailments inhibiting her ability to safely ambulate in-and-out of her home. These ailments include dementia, arthritis, COPD, depression, diabetes, hearing impairments, heart disease, high blood pressure, visual impairments, and kidney problems. Client has expressed the need for assistance with a wheelchair ramp. Due to financial constraints, the household is unable to provide financial assistance to pay for a ramp to be built. Any assistance is greatly appreciated!
St. Clair County Projects
JRHR7 – Client is a 60-year-old disabled male living alone in the Pell City area. He suffers from multiple medical ailments, including neuropathy, psoriasis, history of concussions, and he has a had multiple stomach operations. Client is on monthly fixed-income, so he is unable to pay for the home modifications projects that he is in need of. Requests include a wheelchair ramp and a grab bar. Any assistance is greatly appreciated!
JRHR8 – Client is a 92-year-old man who lives alone in Pell City. He is wheelchair bound and receives some assistance with daily living tasks. He needs assistance with building a ramp so he can safely enter and exit his home. His home has stairs at both the front and back entrances. He has a limited income and cannot afford to pay for the construction of the ramp; therefore, he has asked for assistance with building a ramp for his home. Any assistance is greatly appreciated!
JRHR15 – Client is a 52-year-old man who lives in Ashville. He has health issues which affect his ability to navigate his environment. He needs assistance repairing his ramp, the handrails on his porch, and 2 doors to be installed. He has a limited income and is not able to afford the repairs; therefore, he has asked for assistance with building a ramp for his home. Any assistance is greatly appreciated!
JRHR24 – Client is a 56-year-old female who lives in Margaret with her daughter. She has multiple medical ailments that inhibit her ability to safely move in-and-out of her home. Client has expressed the need for assistance with a wheelchair ramp and handrails in her bathroom. She is on a low fixed income, so she is not able to afford the repairs on her own; therefore, she has asked for assistance with completing these projects. Any assistance is greatly appreciated!
JRHR27 – Client is a 59-year-old male who lives in Ashville with his brother. Both he and his brother suffer from multiple medical ailments that inhibit their ability to safely move in-and-out of the home. Client has expressed the need for assistance with a wheelchair ramp and porch. The household has a limited budget, so they are unable to afford the repairs on their own; therefore, the client has asked for assistance with completing these projects. Any assistance is greatly appreciated!
JRHR28 – Client is an 80-year-old female who lives in Springville with a disabled adult son and daughter. Client’s home is older and in need of updating and repairs. She has requested assistance with some painting. The household has a limited budget, so they are unable to afford the project on their own; therefore, the client has asked for assistance with completing this project. Any assistance is greatly appreciated!
JRHR73 – Client is a 92-year-old female widow living alone in the Steele community. Client suffers from multiple medical ailments that inhibit her safety within her home, as well as safety entering and exiting the home. These medical ailments include: arthritis, dementia, diabetes, and heart complications. She has requested assistance with a wheelchair ramp and a durable medial equipment piece that would assist her with getting in-and-out of the shower. She is on a low fixed income, so she is unable to afford the project on her own; therefore, the client has asked for assistance with completing this project. Any assistance is greatly appreciated!
Shelby County Projects
JRHR12 – Client is a 66-year-old man who lives in Montevallo. He is disabled and has a limited income. He needs assistance with building a ramp at his home. Any assistance is greatly appreciated!
JRHR72 – Client is a 70-year-old widow residing in a very rural part of Sterrett. Client needs a wheelchair ramp to assist her with maintaining her independence, health, and safety. Client is on a fixed monthly income; therefore, she is unable to pay someone to assist her with providing this project. Any assistance is greatly appreciated!
JRHR11 – Client is a 77-year-old female residing in Alabaster. Client informed M4A that her home is very old with multiple issues, but she specifically asked for assistance in repairing/replacing some of her more dilapidated windows. Client is on a fixed-monthly income; therefore, she is unable to pay someone to assist her with her repair needs. Any assistance is greatly appreciated!
JRHR12– Client is a 66-year-old male living alone in the Montevallo community. Client has multiple medical ailments that causes him to experience issues getting in and out of his home. Client is on a fixed-monthly income; therefore, he is unable to pay for someone to assist with building a wheelchair ramp to provide safe entry and exit from his home. Any assistance is greatly appreciated!
JRHR57– Client is a 37-year-old female living with her husband, kids, and mother-in-law in the Montevallo community. Client has several debilitating medical complications that makes entry and exit from her home an arduous task. She has expressed the need for a wheelchair ramp and handrails in the bathroom. The household is on a very tight budget; therefore, she has expressed the need for assistance getting her repairs completed through the help of volunteers. Any assistance is greatly appreciated!
JRHR42– Client is a 50-year-old male living alone in a rural part of Columbiana. Client has expressed the need for assistance with some painting and window repair. He is on a low monthly fixed income; therefore, he has expressed the need for assistance getting her repairs completed through the help of volunteers. Any assistance is greatly appreciated!
JRHR25– Client is a 66-year-old female widow living alone in the Alabaster community. She experiences many medical ailments that inhibit her independence within her home. These ailments include arthritis, heart disease, high blood pressure, visual impairments, back problems, AFIB, and cataracts. Client has expressed the need for assistance with repairing her doors, windows, and installing grab bars in her shower. Due to financial constraints she is unable to pay for the projects to be completed. Any assistance is greatly appreciated!
JRHR30– Client is a 44-year-old male living with relatives in the Alabaster community. He experiences many medical ailments that inhibit his independence within his home. These ailments include an amputation, diabetes, hearing and vision impairments, high blood pressure, paralysis, and renal failure. Client has expressed the need for assistance with hanging a large door and a doorway being widened. Due to financial constraints the family can pay for the supplies, but not the labor. Any assistance is greatly appreciated!
Walker County Projects
JRHR63– Client is an elderly male living with his wife in the Jasper area. Client is on hospice with multiple medical ailments inhibiting his health and safety both in the home, and getting in-and-out of the home. The household has a limited budget, so they are unable to afford a wheelchair ramp on their own; therefore, the client has asked for assistance with completing this project. Any assistance is greatly appreciated! COMPLETED
JRHR2 – Client is a 74-year-old disabled female living with her daughter (caregiver) in rural Walker County. She suffers from multiple medical ailments, including dementia. Client is on monthly fixed-income, so she is unable to pay for the home modifications of lawn maintenance that she is in need of. Requests include a broken window needing replacement, broken doorknobs, lawn maintenance, and a walk-in shower to assist the caregiver with getting her mother in-and-out of the shower. Any assistance is greatly appreciated!
JRHR52 – Client is a 60 year-old disabled female living in the Jasper area. She suffers from multiple medical ailments, including chronic pain caused by her arthritis. Client is on monthly fixed-income, so she is unable to pay for the home modifications that she is in need of. Her requested projects include: repairs to her current wheelchair ramp and safety rails in her bathroom. Any assistance is greatly appreciated!
JRHR23 – Client is a middle-aged male living alone in the Parrish community. Client has expressed the need for assistance with the installation of safety rails in his bathroom and debris cleanup in his yard when needed. Due to medical ailments and financial constraints, the client’s ability to complete these tasks are inhibited. Any assistance is appreciated.
JRHR19 – Client is an elderly male living alone in the Nauvoo community. Client has expressed the need for assistance with bush-hogging around him home. Due to medical ailments and financial constraints, the client’s ability to complete these tasks are inhibited. Any assistance is appreciated.
JRHR31 – Client is a 72-year-old widowed female living alone in the Carbon Hill community. Client has expressed the serious need for assistance with replacing the front door on her residence. Client lives on a very low fixed-income and experiences many medical ailments that inhibit her ability to complete the need on her own; therefore, she is unable to pay for the repair on her own. Any assistance is greatly appreciated.
JRHR33 – Client is a 71-year-old widowed female living with her relative in the Jasper community. Client has expressed the need for assistance with acquiring handrails for her front porch, and potentially a smaller back porch to assist her getting in and out of her home. The household has a low monthly income; therefore, they is unable to pay for the repair on her own. Any assistance is greatly appreciated.
JRHR35 – Client is a 65-year-old male with his relative in the Jasper community. Client has expressed the need for assistance repairing the wood on the deck and ramp of their home. The client and his mother both have multiple medical ailments that inhibit safe entry and exit from their home. Repairs to the existing ramp/deck would improve health and safety for both residents. The family does have some supplies to complete this repair, but they are unable to pay for someone to assist, and the home has a low monthly income. Any assistance is greatly appreciated.
